Please complete the Parent Consent Form found below, you will have an opportunity to upload the signed form when you submit your online application or you can return it to Jan Mauk via email, fax, or at the next meeting.
You are also required to be an approved volunteer of the Puyallup School District. If you aren’t already please complete the PSD online volunteer application here: PSD Online Volunteer Application. (if you did this last year you do not have to apply again, they are good for 2 years. If you are unsure if you have completed one please contact email@example.com.) If you do not have a WA State ID number just put “None/Student” and you can select your current school for the school you will be volunteering at.

Application Requirements: You must be a junior high or high school student in the 2020-2021 school year, commit to a planning meeting on December 2nd from 3:15-4:15, and submit the following application form, Parent Consent Form, and a recommendation letter (for new members). You must also be an approved volunteer in the Puyallup School District.
